Bexacat (Bexagliflozin) Flavored Tablets for Cats are a veterinary medication used for the management of diabetes mellitus in cats. Indications:

Baxacat is indicvated to improve glymic control in otherwise healthy cats with diabetes mellitus not previously treated with insulin.

How it works:

Bexacat (Bexagliflozin) works by inhibiting the sodium-glucose co-transporter 2 (SGLT2) in the kidneys of cats. This mechanism of action helps the kidneys remove excess glucose from the bloodstream and excrete it through the urine. By promoting the excretion of glucose, Bexacat helps lower blood sugar levels in cats with diabetes mellitus. This can lead to improved blood sugar control and reduced reliance on insulin injections in some cases. Additionally, by removing excess glucose from the body, Bexacat may also help manage weight and improve kidney function in diabetic cats.

Always provide the Client Information Sheet with the prescription.

Administer one tablet by mouth to cats weighing 6.6 lbs (3.0 kg) or greater once daily, at approximately the same time each day, with or without food, and regardless of blood glucose level.

Monitoring:

  • Sudden onset of hyporexia/anorexia, lethargy, dehydration, or weight loss in cats receiving Bexacat should prompt immediate discontinuation of Bexacat and assessment for diabetic ketoacidosis, regardless of blood glucose level.
  • During treatment with Bexacat, blood glucose, fructosamine, serum β-hydroxybutyrate (BHBA), serum feline pancreas-specific lipase (fPL), liver parameters, serum cholesterol and triglycerides; and body weight and clinical signs should be routinely monitored.
    • Increasing or persistently elevated feline pancreas-specific lipase or liver parameters should prompt further evaluation for pancreatitis and/or hepatic disease and consideration for discontinuing Bexacat.
    • BHBA is the predominate ketoacid in diabetic ketoacidosis. Bexacat should be discontinued if a notable reduction in BHBA is not observed after initiation of Bexacat, or if BHBA persistently rises after an initial reduction.
    • Cats with increasing or persistently elevated cholesterol and triglyceride levels may be at an increased risk for developing diabetic ketoacidosis or euglycemic diabetic ketoacidosis.
    • Bexacat should be discontinued if poor glycemic control, as described below, develops.
  • During the first 8 weeks after initiation of Bexacat, assessment of glycemic control and clinical improvement should be evaluated.
    • A physical examination, an 8-hour blood glucose curve, serum fructosamine and body weight improvement should be evaluated.
    • Cats demonstrating poor glycemic control, including weight loss, an average blood glucose concentration from an 8-hour blood glucose curve ≥ 250 mg/dL, and/or a fructosamine indicating poor glycemic control, as described above, at 8 weeks.
    • Bexacat should be discontinued, and initiation of insulin considered in cats demonstrating poor glycemic control, as described above, at 8 weeks.
  • Cats may present with diabetic ketoacidosis and a normal blood glucose concentration (euglycemic diabetic letoacidosis). Delay in recognition and treatment of diabetic ketoacidosis and euglycemic diabetic ketoacidosis may result in increased morbidity and mortality.
  • Development of diabetic ketoacidosis and euglycemic diabetic ketoacidosis requires the following actions:
    • Discontinuation of Bexacat
    • Prompt Initiation of insulin therapy
    • Administration of dextrose or other carbohydrate source, regardless of blood glucose concentration
    • Appropriate nutritional support should be promptly initiated to prevent or treat hepatic lipidosis.

Contraindications:

  • Do not use Bexacat in cats with diabetes mellitus who have previously been treated with insulin, who are receiving insulin, or in cats with insulin-dependent diabetes mellitus. The use of Bexacat in cats with insulin-dependent diabetes mellitus, or the withdrawal of insulin and initiation of Bexacat, is associated with an increased risk of diabetic ketoacidosis or euglycemic diabetic ketoacidosis and death.
  • Due to risk of severe adverse reactions, do not use Bexacat in cats with evidence of hepatic disease of reduce renal function.

Animal Safety Warnings

  • Bexacat should not be initiated in cats with:
    • Anorexia, dehydration, or lethargy at the time of diagnosis of diabetes mellitus, as it may indicate the presence of other concurrent disease and increase the risk of diabetic ketoacidosis.
    • An fPL level › 5.3 mcg/L, diagnostic imaging consitent with pancreatitis, a history of pancreatitis, or current clinical signs suggestive of pancreatitis.
    • Laboratory values consistent with diabetic ketoacidosis, including elevated urine or serum ketones, and metabolic acidosis (high anion gap, or decreased bicarbonate, pH or partial pressure carbon diocide [PaCO2] levels).
    • A BHBA › 37 mg/dL, or if BhBA is › 25 mg/dL and the cat has a history of renal disease or metabolic acidosis.
  • Persistent plasma bexagliflozin concentrations and reduce clearance of Bexacat, represented as the presence of plasma half-lives in excess of 24 hours, may result in prolonged clinical effects such as glucosuria and/or euglycemia despite discontinuation of Bexacat in some cats with hepatic disease and/or reduced renal function, including cats with clinically undetectable disease at the time of Bexacat initiation. Reduce clearance of Bexacat may contribute to persistent glucosuria, resulting in an osmotic diuresis and dehydration that requires appropriate hydration support. These cats may require hospitalization, which may be protracted, for sequalae such as diabetic ketoacidosis, euglycemic diabetic ketoacidosis, or hepatic lipidosis.
  • Cats should be screened for urinary tract infections and treated, if indicated, when initiating Bexacat. Treatment with Bexacat may increase the risk for urinary tract infections (see Adverse Reactions). Cats treated with Bexacat should be monitored for urinary tract infections and treated promptly. Consider discontinuation of Bexacat in cats with recurrent urinary tract infections.
  • Bexacat may cause increased serum calcium concentrations. Bexacat should be discontinued in cats with persistent increases in serum total calcium or ionized calcium because of increased risk of forming calcium containing uroliths (see Adverse Reactions).
  • Long term use of Bexacat may increase the risk of urothelial carcinoma (see Adverse Reactions).
  • Keep Bexacat in a secure location out of reach of dogs, cats, and other animals to prevent accidental ingestion or overdose.

Precautions:

  • Bexacat should be discontinued in cats who develop diarrhea unresponsive to conventional therapy
  • Consider temporary discontinuation of Bexacat in cats during times of decreased caloric intake, such as surgery or decreased appetite, as administration of Bexacat in these cats may increase the risk of diabetic ketoacidosis or hepatic lipidosis.
  • The osmotic diuretic effect of Bexacat may contribute to inappropriate urination in some cats (see Adverse Reactions)
  • Polyphagia as a compensatory response to caloric wasting from glucosuria may persist in up the 80% of cats, despite evidence of adequate glycemic control, and may lead to progressive weight gain.
  • Approximately 20-30% of cats may have persistent polyuria and/or polydipsia secondary to Bexacat-induced osmotic diuresis and may be a risk factor for dehydration-associated diabetic ketoacidosis.
  • The safety of Bexacat in breeding, pregnant, and lactating cats has not been evaluated.

Storage:

Bexacat should be stored at tum temperature 68 too 77 °F (20 to 25 C°).

A Godsend treatment for feline diabetes,

What a relief for my cat

Bexacat has been a Godsend for my cat. He was just recently diagnosed with diabetes. Prior to the medication, he started having a wobbly walk, which seems to make him uncomfortable. But, now he's back to his old self again, and I am a very happy cat parent.

The Bexacat drug is wonderful. My tortoise was recently diagnosed with diabetes, and given her attitude, insulin would not have been viable.

This website or at least the people behind it are terrible. I put the drug on monthly auto ship, believing that it would get me an 80% discount. My vet is not really responsive to refill requests, so I call EPRx to ask them to request the refill sooner, as I ran out of meds twice while waiting for the vet to issue the refill.

Instead of changing to refill day of the month, they put me on 2 week refills, Then the price went up about $20 a month.

Called in again, to get the schedule sorted out now that i had a 3 month supply sitting around and was told that the pricing was an introductory price, not a permanent one, and that I should come off auto ship and call in once a month for the best price.

So I did that. Once I finally used up the extra refills that the schedule screw up left me with, I called in, requesting the improved price. Was told that there was a manufacturer price increase, and the price was the price. Lie #1.

Ordered a refill

Went googling, found another place selling it 40% cheaper. Called back to try to get the price fixed.

Got offered a price match deal, but would need a manager to authorize it, was told a manager would call back. Lie #2.

Manager never called back. Called in again. Was told that manufacturer insisted on a minimum final sale price, and that the pharmacy I referenced for the price match wasn't a legitimate pharmacy, but they would give me 50% off because I never got a call back. Lie #3 or 4.

Got an email pestering me to give a review. Checked credit card, never got the 50% credit, So here I am writing the review. I'll call back and see about getting Lie #4 or 5.

We found out our cat Kiki was diabetic abruptly while juggling our toddler and finding out we were expecting baby number two. The idea of having to give two injections a day for the rest of Kiki’s life became very overwhelming. We then found out about this new medication called Bexacat which thankfully worked wonders for our cat and makes it easy to function day to day and to even travel.

My cat recently became diabetic. She was formerly semi feral. She’s very sweet and affectionate but there’s not a snowball’s chance in hell that she would let me check her blood glucose or give her insulin injections. Bexacat is like a miracle! Her blood glucose is under control and I am able to crush the tablet and mix it in her food. The cost is not inexpensive but Entirely Pets has the best price and very quick shipping

I absolutely love this product because it is helping my cat. I have seen a drastic change in her health since she started taking it a month ago. She just finished her first container. I started to notice a change in her after a couple of weeks of taking the pills. It has also been very easy to administer to her. I’ve been crushing it and mixing it in her can food once a day. I requested to have this ordered canceled but never received a reply. They shipped the order and are now telling me I can’t return it. I did an online chat (I have the chat history) and they said they would have management contact me. Newly diagnosed diabetic cat

Having a newly diagnosed 13 year old cat, i was completely uncomfortable with the thought of having to inject him twice a day with insulin. Fortunately after researching alternatives to insulin, I found Bexacat, discussed it with my veterinarian and we decided to try it. He's been on it a week and so far so good. Awaiting results of a first blood test but the ease of giving him one crushed up pill in his food per day opposed to two injections of insulin per day is a real plus. Hopefully the test results will prove it is working.

Effective feline diabetes treatment

We are cautiously optimistic that our sweet 12 yr old boy will continue to improve with this fairly new medication. Sudden onset diabetes took us by surprise. Thankfully, his other organs weren't effected because we caught it so quickly. He had neuropathy in his back legs and one day just couldn't walk. We took him immediately to the vet and she knew right away the diagnosis. Ran all the necessary tests and confirmed. Took a few days to get meds approved and a couple of more to receive them. Meds require a follow up 3 - 4 days from administering of first dose. My boy's initial bloodwork number was in the mid-300 range. After 4 doses, his # dropped to 93. He is now walking much better and even making his way up/down the stairs. His personality is back, which makes us happy. Praying for the progress to continue!! Thank you Elanco!!

My cat loves it!

Imagine my surprise when I didn't have to try to force my cat to eat this life-saving medication! She thinks it's a treat. Thank goodness for the flavoring on it. Already, after 3 or 4 days, she's acting a little playful again. I'm so glad I never had her on insulin so I was able to put her on this medication ☺️❤️

Excellent Medication

Our cat was loosing weight and didn’t seem like himself. Naturally we took him to the vet and the culprit was diabetes. We were fearful when the vet informed us our furbaby would need insulin injections twice daily. I remembered reading about the medication in Bexacat during on of my research days. We suggested before our cat started injections we wanted to try Bexacat. Our vet hadn’t even heard of the medication but wrote the prescription. I searched online and ended up at Entirely. Order process was a breeze and the medication arrived in 8 days or so. I immediately took my cat off dry foods which contain 30-50% carbohydrates and only gave can foods (fancy feast & Purina Friskies until the medication arrived. The medication was packaged well upon arrival and I received multiple updates during the shipping process. I immediately feed my furbaby and gave him one pill. He loves the taste of the medication and it has a food scent to it as well. My cats sugar levels decreased from 360 to 87 in one day. My cat is back to being energetic and his appetite has increased. At the weekly follow up the vet was astounded by his response to the medication and now has brochures and information about it at his office. Of course he’s following our case closely since he’s never prescribed Bexacat but so far he’s sold on it’s effectiveness and happily so are we!
