How It Works:
Gentamicin sulfate eliminates bacteria by disrupting their protein production. Clotrimazole targets yeast and fungal organisms commonly responsible for otitis externa, such as Malassezia pachydermatis. Mometasone furoate, a synthetic corticosteroid, reduces inflammation, redness, and itching caused by allergic reactions or chronic ear infections.
Indications:
Mometamax is indicated for the treatment of otitis externa in dogs caused by susceptible strains of fungi and bacteria, including:
- Malassezia pachydermatis (yeast)
- Pseudomonas spp., including P. aeruginosa
- Coagulase-positive Staphylococci
- Enterococcus faecalis
- Proteus mirabilis
- Beta-hemolytic Streptococci

Dosage & Administration:
Before using Mometamax Otic Suspension, clean and dry your dog’s external ear canal thoroughly. Ensure the eardrum (tympanic membrane) is intact before administering the medication.
- Dogs under 30 lbs: Instill 4 drops once daily into the affected ear(s) using the 7.5 g, 15 g, or 30 g bottle, or 2 drops from the 215 g bottle.
- Dogs 30 lbs and over: Instill 8 drops once daily into the affected ear(s) using the 7.5 g, 15 g, or 30 g bottle, or 4 drops from the 215 g bottle.
Treatment should continue for 7 consecutive days unless otherwise directed by your veterinarian.
Caution:
Federal law restricts this drug to use by or on the order of a licensed veterinarian.
Contraindications:
- Discontinue use if hypersensitivity to any ingredient occurs.
- Do not use in dogs with ruptured eardrums.
- Avoid concurrent use with drugs known to cause ototoxicity.
Warnings:
- Temporary hearing loss or deafness may occur in sensitive dogs, especially geriatric patients.
- Discontinue use if vestibular or auditory dysfunction occurs and flush ears with a non-ototoxic solution.
- Corticosteroids may cause birth defects in animals if used during pregnancy (e.g., cleft palate, limb deformities).
- Late pregnancy use may induce premature labor, fetal death, or complications during birth.
Precautions:
- Confirm the tympanic membrane is intact before application to prevent inner ear damage.
- Use beyond the 7-day period may delay healing of the ear canal.
- Discontinue use if resistant bacteria or fungi overgrow during treatment.
- Avoid ingestion. Topical corticosteroids may lead to systemic effects such as fluid retention, increased urination, or immunosuppression.
- Wean pets gradually if they have recently been treated with corticosteroids to avoid adrenal suppression.
Possible Side Effects:
Gentamicin: Risk of nephrotoxicity, vestibular, and cochlear damage if used improperly or on damaged skin.
Mometasone: May cause liver enzyme elevations, appetite changes, increased thirst/urination, and in rare cases, signs of Cushing's syndrome.
Clotrimazole: Rarely causes skin irritation, redness, itching, or swelling.
General Observations: In clinical trials, less than 1% of dogs experienced side effects such as ataxia, proprioceptive deficits, increased water consumption, inflammation of the pinna, or diarrhea.
Storage Instructions:
- Store between 2°C and 25°C (36°F and 77°F).
- Shake well before each use.

Tell your veterinarian if your dog is pregnant, allergic to any of the ingredients in Mometamax Otic Suspension, has perforated eardrums, or is currently taking any other medications, including over-the-counter medicines, vitamins, minerals, and herbal products.
Shake well before use. The usual dose for dogs weighing less than 30 lbs is 4 drops into the ear canal once a day. For dogs weighing 30 lbs or more, place 8 drops into the ear canal once a day. Therapy should continue for 7 consecutive days. Avoid getting this medication in your pet's eyes, nose or mouth.
Side effects associated with this medication are related to the individual ingredients. Gentamicin, while absorbed poorly from the skin, when applied topically for long periods of time to large wounds, burns or denuded skin has a potential for reversible or irreversible ear and kidney toxicity. Hearing loss (usually temporary) may occur. Discontinue this medication and consult with your veterinarian if hearing loss, head tilting or dizziness are noticed. Redness of the skin, stinging, blistering, peeling, fluid retention, itching, hives and general irritation of the skin have been observed occasionally in humans following the use of clotrimazole. Other side effects may also occur. Talk to your veterinarian about any side effect that seems unusual or bothersome to your pet.
Give the missed dose as soon as you remember. If it is nearly time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
If you suspect that your pet has received an overdose of Mometamax Otic Suspension, consult your veterinarian or emergency veterinary clinic immediately.
Avoid using any other medications in your pet's ear unless instructed to by your veterinarian. Do not use Mometamax Otic Suspension in dogs allergic to it or any of the individual ingredients. Use with caution in pregnant animals. Do not use in dogs with perforated eardrums. Discontinue use of this medication if hearing loss is noted during treatment. Use of this product beyond the recommended dose of 7 days may result in delayed wound healing.
Avoid using other ear medications without first talking to your veterinarian. Do not use concurrently with other corticosteroids or aminogylcoside antibiotics. Drugs other than those listed may also interact with Mometamax Otic Suspension. Talk to your veterinarian or pharmacist before taking or using any other prescription or over-the-counter medicines, including vitamins, minerals, and herbal products.

I ordered the med, my vet approved the Rx 3 days later. I kept getting emails saying they were contacting my vet for approval. I finally called and she found the approval right away that had been sitting for days. I told her this was a med that I needed ASAP. She said they would fill it that day, which it looks like they did. I was also told I would get expedited shipping at no cost due to their screw up. I did not get expedited shipping....it came priority USPS. I will not order Rx's from them again. This med works well but you have to watch or hearing loss in your dog caused by gentamicin toxicity.
