Description

Piroxicam in veterinary medicine is most often prescribed to treat certain types of cancers, such as colon, prostate, and bladder cancer. This drug is used off label and discussions with your veterinarian are highly recommended. It can also be combined with opioid analgesics to reduce discomfort in oncology patients, while reducing the drowsiness commonly associated with those drugs.

How It Works

Piroxicam belongs to the class of non-steroidal anti-inflammatory drugs (NSAIDs). Piroxicam is a nonselective cyclooxygenase (COX) inhibitor with inhibitory effects on both COX-1 and COX-2. COX-1 produces prostaglandins that regulate homeostasis, allowing the kidney to respond to hypotension and protect the GI tract. COX-2 produces the prostaglandins that are increased in the presence of inflammation. COX-2 is upregulated in many types of tumors, including nasal epithelial tumors, mammary tumors, colorectal tumorsoral squamous cell carcinomaoral melanoma, prostatic carcinoma, transitional cell carcinoma (TCC) of the urinary bladder and osteosarcoma.

Carefully consider the potential benefits and risks of Piroxicam Capsules, USP and other treatment options before decidign to use Piroxicam Capsules, USP. Use the lowest effective dose for shortest duration consistent with individual patient treatment goals.

After observing the response to initial therapy with Piroxicam Capsules, USP, the dose and frequency should be adjusted to suit an individual patient's needs.

For the relief of rheumatoid arthritis and osteoarthritis, the recommended dose is 20 mg given orally once per day. If desired, the daily dose may be divided. Because of the logn half-life of Piroxicam Capsules, USP, steady-state blood levels are not reached for 7-12 days. Therefore, although the therapeutic effects of Piroxicam Capsules, USp are evident early in treatment, there is a progressive increase in response over several weeks and the effect of therapy should not be assessed for two weeks.

Piroxicam Capsules, USP is indicated

  • For relief of the signs and symptoms of osteoarthritis.
  • For relief of the signs and symptomms of rheumatoid arthritis.

Contraindications

Piroxicam Capsules, USP is containdicated in patients with known hypersensitivity to piroxicam. Piroxicam Capsules, USP should not be given to patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs. Severe, rarely fatal, anaphylactic-like reactions to NSAIDs have been reported in such patients (see Warnings: Anaphylactoid Reactions and Precautions: Preexisting Asthma).

Piroxicam Capsules, USP is contraindicated for the treatment of peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery (see Warnings).

Piroxicam is a non-steroidal anti-inflammatory medication (NSAID). It is a non-selective cyclooxygenase inhibitor (COX) with inhibitory effects on COX-2 and COX-1. COX-1 produces homeostasis-regulating prostaglandins, which allows the kidneys to protect the gastrointestinal tract and respond to hypotension. COX-2 produces the prostaglandins that increase when inflammation occurs. COX-2 is upregulated in some tumor types, such as osteosarcoma, urinary bladder transitional cell carcinoma (TCC), prostatic carcinoma, colorectal tumors, oral squamous cell carcinoma, oral melanoma, mammary tumors, and nasal epithelial tumors.

Potential adverse reactions include:

  • Livery injury due to liver enzyme elevation (like other types of NSAID)
  • Impaired blood clotting resulting from blood platelet effects
  • Decreased kidney function and sudden, severe kidney injury (papillary necrosis)
  • Ulcers, especially when used in conjunction with phenylbutazone, aspirin, or corticosteroids like prednisone or cortisone
  • Peritonitis caused by intestinal perforation
  • Loss of appetite
  • Vomiting
  • Tarry or dark stools caused by intestinal bleeding
  • Intestinal bleeding and ulcers
  • Stomach bleeding and ulcers

Some of these reactions may be life-threatening and can occur suddenly.

  • Review your pet's other medications with their veterinarian to determine if those drugs might interact with Piroxicam. These medications include ACE inhibitors (quinapril, benazepril, and enalapril) or diuretics. Piroxicam could interact with other drugs.
  • Use cautiously in animals with heart failure.
  • Avoid use in animals with a history of high blood pressure, intestinal lining (mucosa) perforation, stomach perforation, inflammation, or bleeding.
  • Do not administer to animals with a history of allergy or hypersensitivity to the medication.
  • Keep out of reach of animals and children.
Contact your closest animal hospital.
Piroxicam should be avoided in pets with allergies to aspirin or other NSAIDs, history of GI ulcer disease, bleeding disorders, or severely compromised cardiac function.
